Our Application and Direct Debit Forms

 
Please click here to download our application form. The form is a short MS-Word document, to which we ask you to attach your curriculum vitae at the end. When completed, you should send the combined document to your sponsor who will then liaise with your seconders, the Clerk and Membership Chairman.

We manage all payments from members by direct debit. Please therefore click here to download a Direct Debit Mandate which should be printed and then completed by hand and returned to The Clerk at our office address.
